One-time Enrollment Fee: $500 / person​
Enrollment fee includes an initial comprehensive laboratory evaluation. Included is your initial History & Physical which can run 2-4 hours where we review your past medical history, surgical history, medications, allergies, and current treatment plans.
You will receive a summary H&P which you should keep with you and provide to your other medical providers. For those seeking a more in-depth initial evaluation, genetic testing can be performed during the intake for an additional fee. These results are yours forever and you can continue work with the results even if you leave the practice.
Help with weight loss or HRT is included in membership at no additional cost
Re-enrollment fee: $800 / person​
We believe this membership model is the best solution for quality primary care. If someone wants to cancel their membership and then come back later, we may accept them, but there will be a re-enrollment fee applied.
